Puppies, Toy Breeds and Hypoglycemia in Dogs
If you see the symptoms listed below such as weakness or listlessness then your puppy may be suffering from hypoglycemia.
If you puppy or toy breed isn't eating then you can try feeding some Nutri-cal off your fingers. It is a malt paste filled with vitamins and sugar designed to be highly palatable to your dog.
If the hypoglycemia persists, it could result in a medical emergency, particularly if you see severe symptoms such as seizure and collapse.
Symptoms of Canine Hypoglycemia
Symptoms of hypoglycemia in dogs include:
* Stupor, disorientation, reduced level of consciousness
* Coma
* Seizures - particularly in dogs 4 years of age or older
* Weakness
* Anxiety
* Severe depression
Treatment of Canine Hypoglycemia
Treatment of hypoglycemia involves getting your dog the glucose to reverse the low blood sugar levels. One approach to try is to feed your dog small meals throughout the day vs. fewer large meals. A blood test to monitor glucose levels will be helpful in arriving at a diagnosis of the condition and if a change in feeding habits corrected the condition.
If you need to take your dog to the veterinary hospital the attending veterinarian will warm your puppy and immediately check blood sugar levels. Intravenous glucose (dextrose) will be provided and inserted directly into the blood stream.
Your veterinarian will not release your dog until your dog is behaving and eating normally for a 24 hour period. Glucose comes in many forms: * Oral monosaccharides (glucose): absorbed through the mucous membranes. * Dextrose: can be given through intravenous feeding (tube into the body( or orally.
In the case of emergency you can use:* Karosyrup
* Honey or nutri-cal (ask your vet)
* Fruit Juices
* Colas (Pepsi, Coke)
* Gatorade
For toy breeds or small dogs place a small amount of Karo syrub on the gums. The gums will absorb the sugar. If your puppy or dog does not quickly respond then rush him or her to the veterinarian.

Giardia

What are the signs of a Giardia infection?

Most infections with Giardia are asymptomatic. In the rare cases in which disease occurs, younger animals are usually affected, and the usual sign is diarrhea. The diarrhea may be acute, intermittent, or chronic. Usually the infected animals will not lose their appetite, but they may lose weight. The feces are often abnormal, being pale, having a bad odor, and appearing greasy. In the intestine, Giardia prevents proper absorption of nutrients, damages the delicate intestinal lining, and interferes with digestion.

How do we diagnose giardiasis?

Giardiasis is very difficult to diagnose because the protozoa are so small and are not passed with every stool. Tests on serial stool samples (one stool sample every day for three days) are often required to find the organism. Special diagnostic procedures, beyond a routine fecal examination, are necessary to identify Giardia. The procedures we use to identify roundworms and hookworms kill the active form of Giardia and concentrate the cyst form.
To see the active form, a small amount of stool may be mixed with water on a microscope slide and examined under high magnification. Because these forms have flagella, you can see them move around on the slide. The active forms are more commonly found in loose stools. If you ever have the opportunity to see the active form of Giardia under the microscope, take it! It is an interesting-looking creature. It is pear-shaped and its anatomy makes it look like a cartoon face, with eyes (which often look crossed), nose, and mouth. Once you see it, you will not forget it.
Cysts are more commonly found in firm stools. Special solutions are used to separate the cysts from the rest of the stool. The portion of the solution that would contain the cysts is then examined microscopically.
In spring, 2004, a diagnostic test using ELISA technology became available. This test uses a very small fecal sample, and can be performed in 8 minutes in a veterinarian's office. It is much more accurate than a fecal examination.

If we find Giardia, how do we treat it?

Here we go again; treatment is controversial too. There is a question about when to treat. If Giardia is found in a dog without symptoms should we treat the animal? Since we should not know if G. canis can infect man, we often err on the side of caution and treat an asymptomatic infected animal to prevent possible transmission to people.
If we highly suspect infection with Giardia, but can not find the organism, should we treat anyway? This is often done. Because it is often difficult to detect Giardia in the feces of dogs with diarrhea, if there are no other obvious causes of diarrhea (e.g.; the dog did not get into the garbage several nights ago) we often treat the animal for giardiasis.
There are several treatments for giardiasis, although some of them have not been FDA-approved for that use in dogs. Fenbendazole is an antiparasitic drug that kills some intestinal worms and can help control giardia. It may be used alone or with metronidazole. Metronidazole can kill some types of bacteria that could cause diarrhea. So if the diarrhea was caused by bacteria, and not Giardia, the bacteria can be killed and the symptoms eliminated. Unfortunately, metronidazole has some drawbacks. It has been found to be only 60-70% effective in eliminating Giardia from infected dogs, and probably is not 100% effective in cats, either. It can be toxic to the liver in some animals. It is suspected of being a teratogen (an agent that causes physical defects in the developing embryo), so it should not be used in pregnant animals. Finally, it has a very bitter taste and many animals resent taking it – especially cats.
Quinacrine hydrochloride has been used in the past, but is not very effective and can cause side effects such as lethargy, vomiting, anorexia, and fever.
But now we come to yet another unknown. It is possible these treatments only remove the cysts from the feces but do not kill all the Giardia in the intestine. This means even though the fecal exams after treatment may be negative, the organism is still present in the intestine. This is especially true of the older treatments. So treated animals could still be a source of infection for others.

What are coccidia?

Coccidia are small protozoans (one-celled organisms) that multiply in the intestinal tracts of dogs and cats, most commonly in kittens and puppies less than six months of age, in adult animals whose immune system is suppressed or in animals who are stressed in other ways (e.g., change in ownership, other disease present).

In cats and dogs, most coccidia are of the genus called Isospora. Isospora canis and I. ohioensis are the species most often encountered in dogs. Regardless of which species is present we generally refer to the disease as coccidiosis. As a puppy ages it tends to develop a natural immunity to the effects of coccidia. As an adult it may carry coccidia in its intestines, shed the cyst in the faeces, but experience no ill effects.

How are coccidia transmitted?

A puppy is not born with the coccidia organisms in its intestine. However, once born, the puppy is frequently exposed to its mother's faeces and if the mother is shedding the infective cysts in her faeces then the young animals will likely ingest them and coccidia will develop within their intestines. Since young puppies, usually those less than six months of age, have no immunity to coccidia, the organisms reproduce in great numbers and parasitise the young animal's intestines. Oftentimes this has severe effects.

From exposure to the coccidia in faeces to the onset of the illness is about 13 days. Most puppies who are ill from coccidia are, therefore, two weeks of age and older. Although most infections are the result of spread from the mother, this is not always the case. Any infected kitten or puppy is contagious to other puppies. In breeding facilities, shelters, animal hospitals, etc., it is wise to isolate those infected from those that are not.

What are the symptoms of coccidiosis?

The primary sign of an animal suffering with coccidiosis is diarrhoea. The diarrhoea may be mild to severe depending on the level of infection. Blood and mucous may be present, especially in advanced cases. Severely affected animals may also vomit, lose their appetite, become dehydrated, and in some instances, die from the disease.

Most infected puppies encountered by the authors are in the four to twelve week age group. The possibility of coccidiosis should always be considered when a loose stool or diarrhoea is encountered in this age group. A microscopic fecal exam by a veterinarian will detect the cysts confirming a diagnosis.
What are the risks?

Although many cases are mild it is not uncommon to see severe, bloody diarrhoea result in dehydration and even death. This is most common in animals who are ill or infected with other parasites, bacteria or viruses. Coccidiosis is very contagious, especially among young puppies. Entire kennels may become contaminated, with puppies of many age groups simultaneously affected.

What is the treatment of coccidiosis?

It should be mentioned that stress plays a role in the development of coccidiosis. It is not uncommon for a seemingly healthy puppy to arrive at its new home and develop diarrhoea several days later leading to a diagnosis of coccidia. If the puppy has been at the new home for less than thirteen days, then it had coccidia before it arrived. Remember the incubation period (from exposure to illness) is about thirteen days. If the puppy has been with its new owner several weeks, then the exposure to coccidia most likely occurred after the animal arrived at the new home. The authors merely point this out as they have been involved in legal cases as to who was responsible for the cost of treatment, the breeder or new owner. Usually coccidia was present only to surface during the stressful period of the puppy adjusting to a new home.

Fortunately coccidiosis is treatable. Drugs such as sulfadimethoxine (Albon) and trimethoprim-sulfadiazine (Tribrissen) have been effective in the treatment and prevention of coccidia. Because these drugs do not kill the organisms, but rather inhibit their reproduction capabilities, elimination of coccidia from the intestine is not rapid. By stopping the ability of the protozoa to reproduce, time is allowed for the puppy's own immunity to develop and remove the organisms. Drug treatments of five or more days are usually required.

How is coccidiosis prevented or controlled?

Because coccidia is spread by the faeces of carrier animals, it is very important to practice strict sanitation. All faecal material should be removed. Housing needs to be such that food and water cannot become contaminated with faeces. Clean water should be provided at all times. Most disinfectants do not work well against coccidia; incineration of the faeces, and steam cleaning, immersion in boiling water or a 10% ammonia solution are the best methods to kill coccidia. Coccidia can withstand freezing.

Cockroaches and flies can mechanically carry coccidia from one place to another. Mice and other animals can ingest the coccidia and when killed and eaten by a dog, for instance, can infect the dog. Therefore, insect and rodent control are very important in preventing coccidiosis.

A small breed dog runs across the yard chasing a tossed ball. In mid-stride, he yelps in pain and pulls his left hind leg off of the ground. After a second, he continues limping on in three-legged fashion. After ten minutes, the rear leg drops back down to the ground and he uses it normally. This episode occurs maybe once a week. It never really seems to bother him that much – a yip of pain, a short period of lameness, and in a few minutes he is back to his old self. Typically, he is a small or toy breed such as a Lhasa Apso, Pekingese, Pomeranian, Poodle, or Boston Bull.

A luxating patella may affect some animals much more severely. They may hold the leg up for several days and show considerable discomfort. Dogs who have a luxating patella on both hind legs may change their entire posture, dropping their hindquarters and holding the rear legs farther out from the body as they walk. Those most severely affected may not even use their rear legs, walking by balancing themselves on their front legs like a circus act, holding their hindquarters completely off the ground.

Normal knee anatomy

The patella is the bone we know as the knee cap. A groove in the end of the femur allows the patella to glide up and down when the knee joint is bent back and forth. In so doing, the patella guides the action of the quadriceps muscle in the lower leg. The patella also protects the knee joint.

Looking at the lower front portion of the femur (the thigh bone) in a normal dog, you will notice two bony ridges that form a fairly deep groove in which the patella is supposed to slide up and down. These structures limit the patella’s movement to one restricted place, and in so doing, control the activity of the quadriceps muscle.

The entire system is constantly lubricated by joint fluid. It works so that there is total freedom of motion between the structures.

What occurs when the patella is luxated?

In some dogs, because of malformation or trauma, the ridges forming the patellar groove are not prominent, and a too-shallow groove is created. In a dog with shallow grooves, the patella will luxate (jump out of the groove) sideways, especially toward the inside. This causes the leg to 'lock up' with the foot held off the ground.

When the patella luxates from the groove of the femur, it usually cannot return to its normal position until the quadriceps muscle relaxes and increases in length. This explains why the affected dog may be forced to hold his leg up for a few minutes or so after the initial incident. While the muscles are contracted and the patella is luxated from its correct position, the joint is held in the flexed or bent position. The yip is from the pain caused by the knee cap sliding across the bony ridges of the femur. Once out of position, the animal feels no discomfort and continues his activity.

Which dogs are at risk of having a luxated patella?

Smaller breeds of dogs, especially Miniature and Toy Poodles, have the highest incidence of patella luxation. Genetics can play a role.

In certain breeds that have extremely short legs such as the Basset Hound or Dachshund, patellar luxation is thought to be secondary to the
abnormal shape of the
femur and tibia. The curvatures of the bones in these breeds work in conjunction with the forces of the quadriceps muscles to displace the
patella to the inside.
Please do not misunderstand – not all members of these breeds are affected with patellar luxation, only a small portion.

What are the symptoms?

Most dogs are middle-aged, with a history of intermittent (on-again-off-again) lameness in the affected rear leg(s). An affected dog commonly stops and cries out in pain as he is running. The affected leg will be extended rearward, and for a while the dog is unable to flex it back into the normal position.

What are the risks?

Uncorrected, the patellar ridges will wear, the groove will become even shallower, and the dog will become progressively more lame. Arthritis will prematurely affect the joint, causing a permanently swollen knee with poor mobility. Therefore, a good evaluation needs to be done by your veterinarian early in the condition to prevent long-term arthritic crippling.

Treatment for Luxating Patellas

As would be expected, medical therapy has little corrective ability in this disorder and surgery is therefore required and is the treatment of choice. A surgical treatment is not necessary in every individual with this condition.

Surgery can alter both the affected structures and the movement of the patella. The groove at the base of the femur may be
surgically deepened to better contain the knee cap. This procedure is known as a trochlear modification. The knee cap
itself may be "tied down" laterally (on the outside) to prevent it from deviating medially (toward the inside). This is called
a lateral imbrication. The bony protuberance at the site of the attachment of the quadriceps tendon on the tibia may be
cut off and then re-attached in a more lateral position. This is termed a tibial crest transposition. All of these procedure
work well and the type performed depends on the individual case and the clinician. The animal should respond quickly
after surgery and is usually completely recovered within 30-60 days, depending upon the procedure.

Breeding Considerations

Because of the strong genetic relationships, we really feel that animals with this disorder should not be used for breeding. They can still be excellent pets - and those that do require surgery will usually lead perfectly normal lives without any restrictions on activity.
